IPD Billing in Hospitals: Room Rent GST Rules Explained
Hospital room rent is where most billing software quietly goes wrong. The rule is short, but the edge cases — mixed stays, threshold crossings, ICU transfers — are where manual calculation fails. Here is the complete picture for IPD billing.
The ₹5,000 Rule in Plain Language
- Room rent ≤ ₹5,000 per day: GST exempt (0%).
- Room rent above ₹5,000 per day: 5% GST on the entire room charge — not just the excess.
That full-amount rule is the trap. A ₹5,200 room is taxed at 5% on ₹5,200, producing ₹260 — not 5% on the ₹200 excess. This rule has applied since 18 July 2022 and was not changed by the September 2025 GST 2.0 rate rationalisation; confirm the current notification with your CA before changing your tariff.
ICU Is Always Exempt
ICU, CCU and NICU charges are exempt regardless of amount — a ₹15,000/day ICU bed attracts no GST. Critical care is treated as essential healthcare, full stop.
Mixed Stays: Evaluate Day by Day
A five-day admission is not one tax event. Each day is evaluated independently, then consolidated at discharge.
| Day | Room | Charge | Treatment |
|---|---|---|---|
| 1–2 | ICU | ₹9,000/day | Exempt (ICU) |
| 3 | General ward | ₹3,500/day | Exempt (≤ ₹5,000) |
| 4–5 | Private room | ₹6,200/day | 5% of ₹6,200 = ₹310/day × 2 days = ₹620 |
The discharge invoice must show this split explicitly — exempt room days and taxable room days as separate lines, with the correct tax on the taxable portion.
Why 5% Without ITC Matters
The 5% rate is a without input tax credit rate. The hospital cannot claim ITC on the inputs attributable to that supply. Building it into your pricing model matters: a taxable luxury room is genuinely more expensive to provide than the headline rate suggests.
What Your Billing Software Must Do
- Evaluate the threshold per room day, not per bill.
- Hard-code the ICU/CCU/NICU exemption.
- Apply 5% to the full amount the moment the threshold is crossed.
- Consolidate correctly on the discharge invoice with exempt/taxable line separation.
- Flow taxable room rent into GSTR-1 while exempt amounts land in the exemp schedule.
How the Discharge Bill Should Look
| Line | Amount | GST |
|---|---|---|
| ICU, 2 days × ₹9,000 | ₹18,000 | Exempt |
| General ward, 1 day × ₹3,500 | ₹3,500 | Exempt |
| Private room, 2 days × ₹6,200 | ₹12,400 | 5% = ₹620 |
| Nursing and doctor visits | ₹4,800 | Exempt |
| Medicines and consumables used in treatment | ₹7,250 | Exempt (part of treatment) |
Show taxable and exempt lines separately and total the GST at the bottom. Confirm the treatment of medicines and consumables with your CA for your hospital’s billing structure.
FAQ
- Is the ₹5,000 limit per room or per patient? Per room per day, as charged.
- What about a room upgrade mid-stay? Each day follows its own room's rate — another reason day-by-day evaluation is mandatory.
- Are attendant charges taxable? Attendant services billed separately are outside room rent; treat per the nature of the service and your CA's advice.
