TPA & Insurance Billing in Hospitals: A Pharmacy Manager’s Guide
Insurance billing is where hospital revenue goes to wait. Claims stall over missing documents, wrong co-pay estimates and untracked pre-auths. The process is manageable — if it is systematised from admission.
Cashless vs Reimbursement
- Cashless: the TPA settles the hospital directly (subject to approved amounts); the patient pays only co-pay and non-covered items.
- Reimbursement: the patient pays the hospital; the insurer reimburses the patient later. The hospital still owes clean documentation.
The Pre-Auth Workflow
- At admission, record insurer/TPA, policy number and planned treatment.
- Raise the pre-authorization with clinical justification.
- Track approval amount and reference number against the running bill.
- For planned surgeries, raise enhancement requests before the bill crosses the approved amount.
The classic failure: crossing the approved limit mid-stay and discovering it at discharge. Daily charge posting with a pre-auth ceiling visible on the patient dashboard prevents it.
Co-Pay, Deductions and the Art of the Estimate
Co-pay percentages, room-rent caps in policy terms and non-payable items (attendant food, certain consumables) create deductions at settlement. Estimate them at admission, communicate to the family in writing, and reconcile at discharge. Families forgive accurate estimates; they do not forgive surprises.
The Document Trail That Settles Claims
- Claim form and policy copy with ID proof.
- At discharge: complete discharge summary with diagnosis codes.
- Itemised final bill with the exempt/taxable split intact.
- Investigation reports, prescription copies, implant stickers where applicable.
Tracking to Closure
Every claim needs four statuses visible at a glance: Pre-Auth → Approved → Settled → Received, with approved vs claimed vs received amounts. Cashless shortfalls and reimbursement follow-ups then become a weekly worklist, not a memory test.
Common Deductions That Delay Settlement
- Room-rent capping: a policy capped at a lower room category can reduce the payable amount of related charges too — check policy terms at admission.
- Non-payable consumables: gloves, admission kits and similar items are often excluded.
- Missing documents: discharge summary, investigation reports, implant stickers and pharmacy bills must match the final bill.
- Mismatched dates or names between ID, policy and hospital records.
A short pre-discharge check of these four items saves days of back-and-forth with the TPA.
FAQ
- Who bears the shortfall when the TPA pays less? The patient, per the tripartite arrangement — which is why written estimates at admission matter.
- Does GST change because an insurer pays? No. The GST treatment of each hospital service is the same whoever pays. Separately, premiums on individual health insurance policies have been GST-free since 22 September 2025 — that affects the patient's policy cost, not your hospital bill.
- Can claims be tracked per TPA? Yes — and you should: settlement behaviour varies sharply between TPAs.
- What about pharmacy consumption during a cashless stay? It bills per policy terms — some policies allow it, some exclude it. Capture the rule per policy at admission.
